New! Discover Halaxy's latest Patient Forms enhancements

Halaxy’s patient forms are fully customisable and can be used for anything from intake and informed consent to service agreements, clinical tools, and even admin updates like payment or claim details.

With Halaxy, when a patient submits a completed patient form, their patient profile in your account is automatically updated with their details. This removes administration, reduces human errors, and gives you more time to focus on patient care.

Shareable links for patient forms

With our new form links feature, you can quickly and easily create shareable links for any of your patient forms, including intake forms, informed consent forms, and more.

You can use form links in your practice to provide access to any patient form you’ve created in Halaxy, such as an intake form, via an internet connected device like a tablet, via a QR code, or embed them as buttons on your practice website.

Online forms for professional contacts

You can now create patient forms designed to be filled out by a professional contact, such as a GP, specialist, or NDIS case manager, and embed them directly onto your practice website or include them as a link in any communications.

This makes it easy for referring practitioners to quickly and easily complete and submit referrals on their patients' behalf, with any information they submit automatically added to your Halaxy account, including the professional contact’s details.

Streamlined forms preview

You can now quickly preview any patient form in Halaxy, making it easy to see what your patient forms look like and ensure that they function you intended before you share them with patients. To preview a patient form, all you need to do is:

• Go to Settings > General
• Under the section Template Settings, next to Patient forms, click the pencil icon.
• In the list, next to the form you want to preview, click the three dots icon and select Preview.
